Pediatric Care Management
Serenity Pediatrics is proud to offer Care Management services by our specially trained Registered Nurse Care Managers to all patients of the practice. Our RN Care Managers work closely with our clinical team regarding your child’s healthcare needs. Together, the care managers partner with you, your child’s pediatrician, and your child’s entire health care team, including specialists, to achieve overall health goals. For example, a care manager may work with families and children/adolescents who have health concerns such as healthy weight, Asthma, Attention Deficit Hyperactivity Disorder and/or Behavioral Health Needs. They also help families coordinate care between any specialists that your pediatrician has referred your child to. They help children, adolescents and parents establish goals that can lead to an improvement in their health and quality of life. This may be achieved by:
- Reviewing your child’s health information
- Providing education on specific health issues
- Helping to schedule specialist appointments
- Obtaining progress notes from specialists
- Helping to create personalized goals
- Connecting the patient and family with available community resources, such as help with prescriptions, transportation or insurance
- Helping you understand the medications your child is taking, which includes dosage, how often to take them and the purpose of the medication
