Pediatric ADHD Evaluation
Serenity Pediatrics offers initial evaluations and management appointments for patients who are diagnosed with attention-deficit hyperactivity disorder (ADHD) and attention-deficit disorder (ADD). These appointments are scheduled as an initial evaluation, followed by regular follow-up appointments throughout the course of treatment.
Patients being treated for ADD or ADHD at Serenity Pediatrics are enrolled into our Care Management program and followed by one of our RN care managers. This helps us to ensure that the patient is staying up to date with their follow-up appointments and receiving the care and support they need.
The first step in diagnosing ADHD is to schedule an appointment for an initial evaluation with one of our physicians. After this appointment, you will likely be asked to complete the forms linked below.
Please note that a total of four (4) completed forms are required in order to complete the evaluation: two (2) parent informant forms and two (2) teacher informant forms. Once these are completed, please return them to our office and one of our nurses will reach out to you to schedule a follow-up.
