We are currently accepting new patients with the following insurance plans: Blue Cross & Blue Shield of Michigan, Blue Care Network, Cigna, and Priority Health. Please review our updated vaccination policy to ensure that you be in compliance prior to proceeding with your registration.

Step 1
Please Confirm That We Are Currently Accepting New Patients With Your Insurance Plan and Confirm That Your Insurance Plan is In-Network
- You may need to contact your insurance company. If there are any questions, please contact our office at 248-533-0000 and ask to speak with a member of the billing team.
- Please make sure one of our Serenity Pediatrics physicians is listed as your Primary Care Provider (PCP).
Step 2
To save on waiting time, please fill out the two (2) forms below. When registering more than one child, please fill out registration for each child individually. All registration parts should be completed with the child’s information and not the parent’s information.
Please allow 3 business days from time of form submission for processing and verification by our office. Once we have received all registration information, we will contact you to schedule an appointment.

Upon scheduling your first appointment, you will receive a text message link 7 days in advance to check-in and complete pre-visit medical history forms prior to your appointment. We recommend completing these forms prior to arriving to the office.
