Patient Centered Medical Home (PCMH)
Serenity Pediatrics is proud to be designated as a Patient-Centered Medical Home (PCMH). PCMH is a model in which a patient’s treatment is coordinated through the Primary Care Physician (PCP) to ensure patients receive the necessary quality of care when and where they need it. It is a trusting partnership between a physician-led health care team and an informed patient.
As a PCMH practice, patients and families may notice that:
- We ask you what your goal is or what you want to improve in your child’s health
- We ask you to help us plan your child’s care and to let us know if you think you can follow that plan
- We remind you when tests, procedures, or appointments are due so that you can receive the best quality care
- We may ask you to have blood tests done before your child’s visit so that the doctor has the results at your child’s visit
- We are always exploring methods to care for you and your child better
In addition, our Serenity Pediatrics providers trust in our patients to:
- Tell us what you know about your child’s health and any illnesses
- Follow the care plan that is agreed upon- or let us know why you cannot so that we can try to help or change the plan if appropriate
- Tell us what medications your child is taking and ask for a refill at your visit when needed
- Let us know when you see other doctors and what medications they put your child on or changed
- Seek our advice before you see other physicians
- Learn about your insurance and what it covers
- Learn about wellness and how to prevent diseases
