Our office is open Monday - Thursday 8:00AM - 5:00PM (with additional after-hours appointments from 5:00PM - 6:00PM on Monday - Thursday), Friday 8:00AM - 4:30PM, and Saturday 8:00AM - 12:30PM.
Appointments can be made by calling our office at (248) - 533- 0000 during regular business hours.
Our phone lines open at 8:00AM daily. Our emergency on-call service for urgent after hours concerns begins at 6:00PM on Monday - Thursday, at 4:30PM on Friday, and after 12:30PM on Saturdays.

Please allow our office 48 hours notice for any cancelled or re-scheduled appointments.
Appointments may be cancelled through the patient portal or by calling the office at (248)-533-0000 during regular business hours. Our office provides a phone-call, text message, or email courtesy reminder of your child’s appointment 7 days, 48 hours, & 24 hours in advance of the appointment.
Appointments that are cancelled or rescheduled with less than 48 hours notice may be subject to a $50 fee.
No call/no show appointments may be subject to a $100 fee.
Requests for prescription refills can be made through the patient portal or by calling our office at (248)-533-0000 during regular business hours. When contacting our office for a refill request, please have the medication and dosage requested on hand. For ADHD medication refills, please reference our separate ADHD office policy.

Requests for referrals can be made through the patient portal or by calling our office at (248)- 533-0000 during regular business hours. Depending on your insurance plan, it may be required for your child to be evaluated in office by our providers prior to us generating a referral.

health appraisal & sports physical forms
Health appraisal, sports physical, or any additional forms can be dropped off at our office or emailed to firstname.lastname@example.org. Please ensure that all demographic information including child’s name and date of birth has been completed and that the health history or medical history section has been completely filled out. We cannot fill out or sign your form if these sections are not completed. Please ensure that your child has had their annual well child exam within the last calendar year. We cannot fill out your form if your child has not been seen within the last calendar year. Copies of these forms are available on our website under "Office Forms."

When dropping off or emailing your form, please ensure that you specify how you would like the form to be returned to you. Forms can be emailed, mailed, or picked up at our office.
FMLA paperwork can be dropped off at our office or emailed to email@example.com. Please ensure that all pertinent information has been completed and specify the fax number and individual to whom the completed paperwork should reach.
Please allow our office seven (7) business days to complete FMLA paperwork.
